Appointments
In order to be prepared for your first visit, we require a referral from a provider already involved in your care. Once we receive that referral, we will contact you within 24 hours to schedule your first visit.
You can use MyChart to schedule a follow-up online anytime, or call our clinic during office hours so we can schedule for you.
If you are seeking a second opinion, please call our clinic during office hours.
New Patients
We ask that you arrive 30 minutes before your first visit so we can collect necessary information.
What to Bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
