Schedule your first visit with or without a referral from your primary care provider. Your insurance plan may cover the cost of your visit. Our team will verify your coverage. Please bring these items on the day of your visit:

• Photo ID
• Insurance card
• A list of your medications and the dosage.
• Records that describe your medical and surgical histories
