We look forward to serving you in our clinic. So that we can understand the nature of your visit and schedule appropriate time for you to see a heart care provider, we ask that you call us during clinic hours.

New patients

Please arrive at least 15 minutes before your first visit. If you are scheduled for a diagnostic test, please do not wear lotions or oils.

What to bring

  • Your photo ID
  • Your insurance card
  • Your method of copayment or payment in full if insurance will not cover your visit
  • Any medications you are currently taking, including over-the-counter medicines, in their original containers
  • Any forms you have completed ahead of time using the links below
