Since we are a referral-based clinic, please ask a provider involved in your care to refer you. We will contact you to schedule your visit once we receive your referral information.

If you are an established patient in need of follow-up care, please call us during office hours so we can schedule your visit.

New Patients

We look forward to seeing you in our clinic. Please arrive at your appointment time; you do not need to arrive early.

What to bring

  • Your photo ID
  • Your insurance card
  • Your method of copayment or payment in full if insurance will not cover your visit
  • A list of any medications you are currently taking, including name and dosage
