To schedule an appointment for primary care in our clinic, please call us during office hours.

New Patients

We look forward to serving you! Once you schedule your first appointment, you will receive a link via email to complete new patient paperwork online prior to your visit.

We ask that you arrive 15 minutes prior to your first appointment so we can collect needed information from you.

What to bring

• Your photo ID
• Your insurance card
• Your method of copayment or payment in full if insurance will not cover your visit
• A list of any medications you are currently taking, including name and dosage
• Any forms that you need our office to complete
