Please all us during office hours to schedule an appointment in our clinic.

If you are referred to your clinic, we will contact you to schedule a visit once we receive your referral information.

New Patients

For your first appointment, please arrive 15 minutes prior to your appointment time. You can expect a text message with a link to complete the check-in process before your visit.

What to bring

• Your photo ID
• Your insurance card
• Your method of copayment or payment in full if insurance will not cover your visit
• Comfortable shoes
• Any medical records from previous podiatrists
• A list of any medications you are currently taking, including name and dosage
• Any forms you need our office to complete
