Appointments
So that your first visit can be as productive as possible, we require a referral from a provider already involved in your care. We will contact you to schedule a visit once we receive your referral information.
Established patients wishing to schedule follow-up care can call us during office hours to schedule.
New Patients
We look forward to serving you in our clinic. Please arrive 15 minutes prior to your first appointment time so we can collect any needed information before you see your provider.
What to bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any CDs or files you have with previously completed diagnostic images (CT scans, MRIs, etc.)
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
