Appointments
We are a referral-based clinic. Once we receive referral information from a provider already involved in your child’s care, we will contact you to schedule a visit.
If you are an established patient, please feel free to call us during office hours or via MyChart so we can help you.
New Patients
We look forward to seeing you and your child in our clinic. Please arrive at least 15 minutes before your child’s scheduled appointment. During this time, we will collect any needed information from you and show you to a room where any necessary labs will be drawn, and you will have a comfortable, private place for you and your child to meet with your genetic specialist on a secure video visit.
What to bring
- Your photo ID
- Your child’s insurance card
- Your method of copayment or payment in full if insurance will not cover your child’s visit
- A list of any medications your child currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
