Appointments
So that we can make your child’s first visit as productive as possible, we ask that you receive a referral to our clinic from a provider already involved in your child’s care. Once we receive your child’s referral information, we will contact you to schedule a visit.
Established patients needing follow-up care can schedule through MyChart, or can call us during office hours.
If you are seeking a second opinion, please call us.
New Patients
Please arrive at least 15 minutes before your child’s first appointment.
What to bring
- Your photo ID
- The insurance card covering your child
- Your method of copayment or payment in full if insurance will not cover your child’s visit
- A list of any medications your child is currently taking, including name and dosage
- Any recent scans or test results you have in your possession that are not in the Novant Health system
- Any forms you need our office to complete
- Any forms you have completed ahead of time using the links below
