Appointments
Seeing a provider in our clinic does not require a referral. However, your insurer might require a referral in order for your visit to be covered. Please confirm insurance coverage before scheduling your visit.
You can schedule an appointment using MyChart anytime, or by calling us during office hours.
If another provider involved in your care has referred you to our clinic, we will contact you to schedule a visit once we receive your referral information.
New Patients
Please arrive at least 15 minutes before your first visit. Bring:
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
