Appointments

Call us during office hours so we can schedule your appointment.

Our clinic does not require a referral to be seen, but your insurance might ask for one in order for your vist to be covered. We ask that you confirm insurance coverage before scheduling. 

New Patients

You will receive information about completing new patient paperwork before you see us once your first visit is scheduled. We ask that you arrive at least 15 minutes before your first appointment.

What to bring

• Your photo ID
• Your insurance card, or your child’s insurance card if we are seeing your child
• Your method of copayment or payment in full if insurance will not cover your visit
• A list of any medications you are currently taking, including name and dosage
• Any forms that you need our office to complete
