Here's what to do to be seen at our student health clinic and to get registered:

1. Download and complete PDF application
2. Print completed document
3. Obtain signed immunization documentation
4. Sign any documents requiring student or parent signatures
5. Return the signed and completed documents using one of the following options:
   
   • E-mail scanned and signed copies of the completed documents to HPUStudentHealth@novanthealth.org, or
   • Fax the completed and signed documents to: (910) 754-2009
