I have been going to this facility for several years and have always had perfect care. The staff is always friendly and helpful.

Great experience each time. I had an emergency to get my elderly mother in the day I called and it was handled so well- worthy of me taking time to fill out this form- thank you, that makes all the difference when trying to work through issues with elderly parents - very pleased. My thanks also to PA Lara Hart!

I like that i had no wait to get just an injection. And the PA was quite skillful as I didn’t feel anything but the spray! Excellent!

My appointment was moved back five hours on the morning of the appointment. I found this terribly annoying as I had been waiting since Thursday for my Monday appointment for intense back pain. I tried to move my Monday appointment back to Friday as the pain was increasing each day but the clinic wasis closed on Friday

Simply put, this was the most efficient appt I have EVER had.

I wish there had been an earlier appointment but it was only 2 days later. The staff fro. Start to finish were great. I need some follow up questions answered So I will call on Monday.

The provider was friendly professio nal and caring.

Good experience. Injections really helped. The injection did not hurt. But when she told me I was a “complicated patient “. That kind of made me feel sad.

Lori Ann Hart takes very good care of me. She does excellent injections with very little pain.

Dr. Hart was amazing! She and her staff were incredible.

I feel this is a first class operation run by professionals. I drive an hour to come to see the team! They are professionals!

We were quickly taken back to a room. Then quickly seen by the doctor/nurses. Very nice to have this. I don’t go to the doctor because of the wait time that there usually is. The nurse/PA were very pleasant and took care of me.

The nurses were so nice, everyone of them. I love how they take care of you. It’s like your there family so special. My doctor is awesome I just Love him and his team . I could not if had a better team to take care of my needs from beginning to end . JUST AWESOME!!!! Thanks to all .

The P A I met with was very helpful and honest about my surgery and recovery.

The PA that helps me walked the extra mile for me. Gave a complete explanation of the results of my xrays. The shot that she gave make me feel much better. She is awesome

I went in feeling very hesitant and nervous about my progress after surgery.. But my PA Loriann was very empathetic and put me at ease immediately.

Left my visit exactly as I arrived tried to explain what I experienced with other providers in the past and left feeling way better when I left felt like they weren’t listening to my input
